Important Patient Information

Dear Patient

We are writing to you as a patient who is currently registered at Claypit Street Medical Practice to advise that as from 1st November 2017, Bridgewater Family Medical Practice is taking over the management arrangements for Claypit Street Medical Practice.

Due to ongoing difficulties with providing clinical staffing, Shropdoc took the difficult decision to terminate its contract with Shropshire CCG for patient services at Claypit Street in order to maintain safe services to patients. Shropdoc has been working closely with Bridgewater Family Medical Practice and the CCG to ensure a seamless handover.

Bridgewater Family Medical Practice is a friendly, forward thinking practice committed to improving the health and wellbeing of our patients of all ages. This letter is to welcome you and provides you with information on changes that will apply to you when accessing services from 1st November 2017.

Patients will be able to access services by contacting us Monday to Friday on either Practice number 01948 662097 / 01948 662870 and at either the Bridgewater site or the Claypit site. The opening times are detailed below:

We have arranged to have access to your medical records from both sites so whichever site you contact you will have the information we need to meet your requirements. On the three afternoons that the Claypit Street site is closed, you can collect prescriptions and make appointments at Bridgewater.

To support the changeover patients are advised that it will be necessary for both practices to close at 12 noon on Tuesday 31st October 2017. This is to allow the computer systems that operate the appointments and medical notes to merge together. Urgent care cover will be in place after this time and patients should ring Shropdoc on 0333 222 66 55 in the first instance. The practices will both reopen on 1st November as normal.

You do not need to re-register to be a patient; however you will need to re-register for online services, prescription requesting, appointment booking and medical record viewing via patient access.

We apologise for this inconvenience and will provide extra administration staff to ensure request are dealt with promptly. From 2nd November you will be able to re-register either via the ‘ACCESS ONLINE SERVICES’ link at or directly from and choose the register link. You will not require a letter from the practice as we will enable online registration; our post code is SY13 1QH which will need to be entered and Bridgewater chosen then follow the instructions to register.

We would also like to encourage patients to sign up for the electronic prescription service, which allows prescription requests to be transmitted direct to a pharmacy of your choice without the need for the paper copy being collected from the Practice. Please contact the practice or email us at [email protected] entering the subject heading ‘EPS Nomination Request’ and providing your full name, address, date of birth and the name and town of the pharmacy you wish to nominate.
